You can enroll in one of our programs any time. Please fill out our pre-registration form and/or call us at 239-332-6937 or come to visit us. We are open Monday thru Friday from 8am to 5pm.
The SWFAS Resource Library has materials and information on alcohol, drugs, addiction and more available for FREE, to the community and to other organizations. These materials can be picked up from the Library at our Cape Coral location, or at the McGregor office, or or we can mail them to you.
SWFAS makes every effort to maintain patient confidentiality when securing online data. This includes ensuring the privacy and the security of your personal information. All of your personal information is encrypted as it travels over the Internet. All data entered into our online server is protected by 128-bit SSL data encryption. Data is further protected with state of the art routers, firewalls, and automated virus protection systems. In addition to the technical measures outlined above, we sign a full confidentiality agreement with all new patients upon acceptance into a SWFAS program to assure that all privileged information complies with the Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part II.
The confidentiality of alcohol and drug abuse patient records maintained by SWFAS is protected by federal law and regulations. Generally, SWFAS may not say to a person outside our agency that a patient attends the program or disclose any information identifying a patient as an alcohol or drug abuser, unless:
1.) The patient consents in writing; OR
2.) The disclosure is allowed by a court order; OR
3.) The disclosure is made to medical personnel in a medical emergency or to qualified personnel for research, audit, or program evaluation; OR
4.) The patient commits or threatens to commit a crime either at the program or against any person who works for the program.
Violation of the federal law and regulations by a program is a crime. Suspected violations may be reported to the United States Attorney in the district where the violation occurs.
Federal law and regulations do not protect any information about suspected child abuse or neglect from being reported under state law to appropriate state or local authorities.
